COVID-19 Health Screening

Due to COVID-19, Santa Rosa High School's office
has limited access to guests.

All visitors will be screened with the following questions before approval is granted to enter the office/school:
1. Have you felt unwell/sick the last several days?
2. Do you have any signs or symptoms of cough, sore throat, fever, diarrhea, fatigue, or muscle aches?
3. Have you recently lost you sense of smell or taste?
4. Have you had contact with someone who has been sick or under investiagion for COVID-19?
5. Do you feel like or do you have a temperature? Is it above 100.3?
6. Have you been outside the area in the last two weeks?
7. Have you had visitors to your home outside the area the last two weeks?
In addition, all visitors who are allowed access into the office/school will be required to have their temperatures taken with a non-contact infrared thermometer. Visitors with temperatures less than 100.3 will be permitted to stay.
